Objectives:

To compare the efficacy of CELEBREX® in reducing the incidence of myocardial infarction (MI) and death in patients with primary dysmenorrhea (DM) treated with CELEBREX® in a controlled clinical trial.

In total, 724 patients, of whom 234 were randomized to receive CELEBREX® (n = 241) or placebo (n = 243). The primary outcome was the composite of MI and death from any cause.

Background:

Since the first COX-2 inhibitor, Celebrex (Celebrex®) was FDA-approved in 1988, the incidence of MI and death in primary dysmenorrhea (PM) has been significantly reduced. However, the rates of cardiovascular events are still not as well known, although the incidence of MI and death from any cause has been reduced. This can be attributed to an improved understanding of the mechanisms by which COX-2 inhibitors reduce cardiovascular risks. COX-2 inhibitors are often used to treat acute coronary syndrome (ACS) in patients with severe cardiovascular disease, with or without MI. However, their use has also been associated with an increased risk of stroke, heart attack, and death. This study aimed to compare the efficacy of CELEBREX® in reducing the incidence of MI and death in patients with PM, using a randomized, double-blind, placebo-controlled clinical trial. Methods: This randomized, double-blind, placebo-controlled, double-dummy, controlled clinical trial was conducted between June 2012 and March 2015 in an unblinded setting. Patients with PM treated with CELEBREX® were recruited to the study. The primary end-point was the incidence of MI and death from any cause. Secondary end-points were the composite of myocardial infarction (MI) and death from any cause. Results: In total, 724 patients were randomized to CELEBREX® or placebo. After 3-month therapy, there was a significant reduction in the incidence of MI and death from any cause in the CELEBREX® group. Moreover, the rates of cardiovascular events were significantly reduced in the CELEBREX® group compared with the placebo group.

Findings:

The mean change from baseline in MI, heart attack, stroke, hospitalizations, and death was -0.7 (95% confidence interval [CI], -1.2 to 0.4), -1.0 (95% CI, -0.8 to -0.4), -0.3 (95% CI, -0.2 to 0.3), -0.2 (95% CI, -0.6 to 0.2), and -0.2 (95% CI, -0.8 to -0.4), respectively. There was no significant difference in the mean change from baseline in the other end-points between the CELEBREX® and placebo groups.

CELEBREX® significantly reduced the incidence of MI and death in PM patients treated with CELEBREX® compared with placebo. The reduction in the incidence of MI and death was greater in the CELEBREX® group. However, the overall mortality rates were not significantly different between the groups. This study also provides evidence that CELEBREX® has a favorable safety profile, and has a lower incidence of MI and death in the treatment of PM.
